THE NICU-TO-HOME PROGRAM

One transition, one plan

The first 30 days home,
with a plan.

A STRUCTURED PATHWAY

From discharge preparation
through the first month at home.

Thirty days of structured support.

The Nuravyn NICU-to-Home Transition Program is a structured 30-day pathway designed to help eligible families move from discharge preparation through their first month at home.

Beginning before discharge when possible, families receive scheduled transition support focused on preparedness, education, navigation, and connection to their established healthcare team.

THE 30-DAY JOURNEY

Support designed around
the moments that matter most.

01

Prepare

Before discharge

Understand transition needs, review discharge education, identify barriers, and prepare for the first days at home.

02

Land

First 24–48 hours

Support the shift from hospital routines to home and identify needs that may require added help or provider communication.

03

Settle

Days 3–7

Reinforce education, address transition-related questions, identify barriers, and prepare for appropriate follow-up.

04

Strengthen

Weeks 2–3

Continue scheduled touch points around transition needs, caregiver preparedness, follow-up navigation, and provider-directed concerns.

05

Transition forward

Day 30

Review progress, identify remaining needs, reinforce resources, and move forward with the established healthcare team at the center of care.

Support intensity can be adjusted based on transition needs, allowing families with greater complexity to receive additional touch points within the defined program.

WHAT SUPPORT MAY INCLUDE

Focus on making the discharge plan work at home.

Depending on the family’s needs and program pathway, Nuravyn support may include:

  • Transition-readiness and caregiver-confidence assessments
  • Reinforcement of previously provided NICU discharge education
  • Feeding-plan understanding and preparedness
  • Medication organization and education reinforcement
  • Equipment and supply readiness
  • Appointment and follow-up preparation
  • Identification of practical or logistical barriers
  • Navigation of appropriate healthcare and community resources
  • Scheduled transition check-ins
  • Communication of appropriate concerns back to established healthcare providers

WHO MAY BENEFIT

Designed for NICU families who may need additional transition support.

Program eligibility is established with participating healthcare organizations and may consider factors such as:

Prematurity or prolonged NICU hospitalization

Families transitioning home after an extended or complex neonatal course.

Complex discharge plans

Including multiple medications, specialized feeding plans, home oxygen, equipment, supplies, or multiple specialists.

Higher education or preparedness needs

When families would benefit from additional reinforcement as they begin managing the discharge plan at home.

Follow-up or navigation needs

Including multiple appointments, developmental or NICU follow-up, resource coordination, or logistical barriers.

Caregiver confidence or transition concerns

When the family or clinical team identifies a need for additional support during the transition home.

A DEFINED ROLE IN THE CONTINUUM OF CARE · TRANSITION CARE — NOT A REPLACEMENT FOR MEDICAL CARE

Nuravyn works alongside the baby’s established healthcare team. We reinforce education, support preparedness, help families navigate the transition, identify barriers, and facilitate appropriate communication when needs arise.

Diagnosis, treatment, changes to prescribed medical plans, emergency evaluation, and ongoing medical management remain with the baby’s pediatrician, specialists, and other established healthcare providers.
